How fast can you give Beriplex?
Beriplex® P/N should be administered intravenously at no more than 3 IU/kg/min (max 210 IU/min). A syringe driver should be used if available. The maximum single dose should NOT exceed 5000 IU factor XI. Primary measure of haemostatic efficacy is correction of INR.
How long does Beriplex last for?
The correction of the vitamin K antagonist-induced impairment of haemostasis is reached 30 minutes after the injection and will persist for approximately 6-8 hours.
How is Kcentra dosed?
Kcentra is dosed based on units of Factor IX. The dose is determined by the patient’s pretreatment INR and body weight. *Dosing is based on body weight. Dose based on actual potency is stated on the vial, which will vary from 20–31 Factor IX units/mL after reconstitution.

Beriplex is stable at temperatures between 0 and 25C and should be stored in a fridge in the emergency department. If Beriplex is used then the attached form should be completed and returned to Blood Transfusion to allow re-stocking of the Emergency Department supply.

How do I reconstitute my Beriplex?
Reconstitution and withdrawal must be carried out under aseptic conditions. Reconstitution: Bring the solvent to room temperature. Ensure product and solvent vial flip caps are removed and the stoppers are treated with an antiseptic solution and allowed to dry prior to opening the Mix2Vial package.

Results: 1) PCC corrected INR much faster than FFP. FFP is a slow drip over hours, PCC is a fast drip usually infused over less than an hour, sometimes minutes.
Do you give vitamin K with Kcentra?

When do you repeat INR after FFP?
Check INR 15 minutes after completion of infusion. If INR ≥1.5, administer 2 additional units of FFP IV rapid infusion. Repeat process until INR ≤1.5. May wish to administer loop diuretic between FFP infusions if volume overload is a concern.
How long does vitamin K take to reverse warfarin?
PCC will reverse anticoagulation within minutes of administration; FFP administration can take hours due to the volume required; vitamin K effect takes 12 to 24 hours, but administration of vitamin K is needed to counteract the long half-life of warfarin.
Does Kcentra decrease INR?
In clinical trials, Kcentra decreased the INR to ≤1.3 within 30 minutes in most subjects. The relationship between this or other INR values and clinical hemostasis in patients has not been established [see Clinical Studies (14)]. Reconstitute using aseptic technique with 20 mL of diluent provided with the kit.
