What is PML caused by?

Progressive multifocal leukoencephalopathy (PML) is a disease of the white matter of the brain, caused by a virus infection that targets cells that make myelin–the material that insulates nerve cells (neurons).

What virus causes PML?

Progressive multifocal leukoencephalopathy is a rare infection of the brain that is caused by the JC (John Cunningham) virus. People with a weakened immune system are most likely to get the disorder.

What are the first symptoms of PML?

Symptoms

  • Clumsiness or loss of coordination.
  • Difficulty walking.
  • Facial drooping.
  • Loss of vision.
  • Personality changes.
  • Trouble speaking.
  • Weak muscles.

Who is at risk for PML?

Your risk increases if you have previously taken an immunosuppressant. These are drugs that reduce the activity of your bodies immune system. Your risk increases the longer you have been on Tysabri, especially over 2 years. Your risk increases with a higher titre level.

Which drugs cause PML?

Natalizumab, predniso(lo)ne, (dimethyl) fumarate, fludarabine, rituximab, and brentuximab vedotin were the most common single agents that have been demonstrated to trigger PML (Online Resource 1).

What MS drugs cause PML?

To date, occurrences of PML have been reported in individuals with MS treated with natalizumab (Tysabri®), dimethyl fumarate (Tecfidera™), fingolimod (Gilenya®) and ocrelizumab (Ocrevus™). There is no cure for PML. Treatment includes immediate termination of current medication.

The high prevalence of JCV in urine and in sewage and the stability of the viral particles observed suggests that contaminated water, food, and fomites could be the vehicles of JCV transmission through the oral route.

There is no cure for PML but one-year survival has increased from 10% to 50% in HIV-infected individuals treated with highly active antiretroviral therapy (HAART).

Immunosuppressive medications that have been associated with PML include cyclophosphamide, corticosteroids, mycophenolate mofetil and monoclonal antibodies including natalizumab (Tysabri), rituximab (Mabthera) and alemtuzumab (MabCampath)8.
