How do you dilute CEREBYX?

Prior to IV infusion, dilute Cerebyx in 5% dextrose or 0.9% saline solution for injection to a concentration ranging from 1.5 to 25 mg PE/mL. 1) The loading dose of Cerebyx is 15 to 20 mg PE/kg administered at 100 to 150 mg PE/min.

How do you dilute phenytoin?

For administration by intravenous infusion phenytoin injection should be diluted in 50 – 100 ml of normal saline, and the final concentration of phenytoin in the solution should not exceed 10 mg/ml, the infusion mixture should not be refrigerated.

How is fosphenytoin dosed?

The dose of IV fosphenytoin (15 to 20 mg PE/kg) that is used to treat status epilepticus is administered at a maximum rate of 150 mg PE/min. The typical fosphenytoin infusion administered to a 50 kg patient would take between 5 and 7 minutes.

Can you give fosphenytoin IV push?

Fosphenytoin is a water-soluble disodium phosphate ester of phenytoin that is converted in plasma to phenytoin. Fosphenytoin is compatible with most common i.v. solutions and can be administered safely through the i.m.route. An additional safety factor is the absence of propylene glycol in the fosphenytoin formulation.

What is CEREBYX used for?

What Is Cerebyx? Cerebyx (fosphenytoin sodium) Injection is an anti-epileptic drug, also called an anticonvulsant, used to prevent or control seizures. Cerebyx is used only for a short time, such as 5 days, when other forms of phenytoin cannot be given.

How long does CEREBYX last?

Absorption/Bioavailability: Fosphenytoin is completely converted to phenytoin following IV administration, with a half-life of approximately 15 minutes. Fosphenytoin is also completely converted to phenytoin following IM administration and plasma total phenytoin concentrations peak in approximately 3 hours.

How do you give phenytoin 100mg IV?

100 mg IV every 6 to 8 hours. Doses adjusted depending on plasma levels. If using neat injection, give as slow IV injection (over two minutes) If giving as an IV infusion, a 0.2 micron filter must be used and infusion completed within 1 hour of preparation. Monitor for adverse effects from IV therapy (see above).

What IV fluid is compatible with phenytoin?

It is concluded that 0.9% sodium chloride and lactated Ringer’s injections are suitable diluents for the intravenous administration of phenytoin.

How do you load fosphenytoin?

Loading dose: 18 to 20 mg PE/kg IV Maintenance dose: 2 to 3 mg PE/kg IV dosed two times daily (4 to 6 mg PE/kg/day), though significantly higher dosing may be needed based upon levels. To decrease the risk of adverse reactions such as paresthesias, pruritus, and hypotension.

What is Cerebyx used for?

How long does Cerebyx last?

What is the generic name for Cerebyx?

Generic Name & Formulations: Fosphenytoin sodium (prodrug of phenytoin) 75mg/mL (equivalent to 50mg/mL phenytoin sodium); soln for IV or IM inj.

How do you calculate phenytoin infusion?

If the patient has not already received phenytoin then give: Phenytoin sodium IV 18mg/kg (see Table 1 below)….Table 1 – IV phenytoin loading dose.

Weight (kg) IV Loading Dose (mg) Volume of IV phenytoin (ml) (vial = 250mg/5ml)
45-54 900 18
55-64 1100 22
65-74 1250 25
75-84 1450 29

How is Dilantin IV given?

Because of the risk of local toxicity, intravenous Dilantin should be administered directly into a large peripheral or central vein through a large-gauge catheter. Prior to the administration, the patency of the IV catheter should be tested with a flush of sterile saline.

How do you give IV phenytoin infusion?

Phenytoin Injection BP should be injected slowly and directly into a large vein through a large-gauge needle or intravenous catheter. It must be administered slowly. Intravenous administration should not exceed 50 mg/minute in adults.

Can you give phenytoin with d5w?

Dilantin can be given diluted with normal saline. The addition of parenteral Dilantin to dextrose and dextrose-containing solutions should be avoided due to lack of solubility and resultant precipitation.

How do you adjust fosphenytoin?

A rough guide to making an adjustment to the daily dose that should increase a serum level without leading to supratherapeutic / toxic levels is: If the phenytoin concentration is < 7 mcg/mL, the dose may be increased by 100 mg/day. If the phenytoin concentration is 7-12 mcg/mL, the dose may be increased by 50 mg/day.

What weight do you use for phenytoin?

Initial loading doses of phenytoin are usually 20 mg/kg of the appropriate dosing weight as above. Mini loads for subtherapeutic subsequent levels are typically 10 mg/kg (quick disclaimer: you may see anywhere from 5 – 15 mg/kg given as a mini load. I’ll just use 10 mg/kg moving forward to make things simple).

Why do we load phenytoin?

Initial loading dose of phenytoin for status epilepticus Ensure ECG, blood pressure and respiratory function are monitored throughout the duration of the infusion.

How fast can you give Dilantin IV?

The drug should be injected slowly intravenously at a rate not exceeding 1 to 3 mg/kg/minute or 50 mg/minute, whichever is slower. Continuous monitoring of the electrocardiogram and blood pressure is essential.

How do you dilute Cerebyx?

Preparation Prior to intravenous (IV) infusion, dilute CEREBYX in 5% dextrose or 0.9% saline solution for injection to a concentration ranging from 1.5 to 25 mg PE/mL. The maximum concentration of CEREBYX in any solution should be 25 mg PE/mL.

How much Cerebyx do you give for IV?

Prior to intravenous (IV) infusion, dilute CEREBYX in 5% Dextrose Injection or 0.9% Sodium Chloride Injection to a concentration ranging from 1.5 to 25 mg PE/mL. The maximum concentration of CEREBYX in any solution should be 25 mg PE/mL.

How do you write Cerebyx in MG PE?

CEREBYX should always be prescribed and dispensed in phenytoin sodium equivalent units (PE). The amount and concentration of fosphenytoin is always expressed in terms of mg of phenytoin sodium equivalents (mg PE). Do not confuse the concentration of CEREBYX with the total amount of drug in the vial.

How many milliliters of Cerebyx are in a 2 ml vial?

Cerebyx is marketed in 2 mL vials containing a total of 100 mg PE/2 mL (50 mg PE/mL) and 10 mL vials containing a total of 500 mg PE/10 mL (50 mg PE/mL), for intravenous or intramuscular administration. The concentration of each vial is 50 mg PE/mL.
