What are aphthous ulcers in the colon?

The earliest mucosal lesion of Crohn’s disease is the aphthous ulcer. An aph- thous ulcer is a small, superficial mucosal ulceration that endoscopically has the appearance of a white spot, usually less than 1–2 mm in diameter, surrounded by normal mucosa (Figure 1).

A common cause of isolated colonic ulcers is the use of nonsteroidal anti-inflammatory drugs (NSAIDs), with ulcers in the cecum and right colon. Isolated rectal ulcers are caused by ischemia, solitary rectal ulcer syndrome (SRUS), radiation, or fecal impaction.
